First, it is for women wigs.
Women wigs, we have two situations, if your head is normal, so you can choose normal size, in this way, you don't need to choose base size, only choose color and hair length, then add to cart. Our workers will make medium size cap for you. But you have special requirements of your cap size, now, you need to wear your head with your Soft ruler, just like the following photos:

And when you make order, pls leave a note on remark of you special size. Without special requirement, we will make wigs in medium size.
Second, it is for men toupee.
When you measure your head, pls take more attention, you only need to measure the part you need to wear toupee, not whole head. And pls check following photo:
